Sonic Jam 6 / Super 1998 Sonic 6 (Megadrive)
The title screen looks a bit like the Saturn Sonic Jam. The sprites aren't properly matched up - this isn't an emulator bug, because it happens on a real Megadrive too.

Sonic Jam 6 is actually based on a Megadrive Mario game (which is itself a pirate game). Collecting a mushroom makes Sonic turn from red to blue.

The game physics are poorly programmed, making Sonic very difficult to control.

There are water levels, just like in the original Super Mario Bros.
The ending. No credits or anything - the game just resets itself.
